SUPPORT

Information & Referral

  • Personalized assistance for older adults (60+) and people with disabilities, and their families, friends, caregivers, advocates and other representatives.
  • Comprehensive information on community resources and public programs.
  • A detailed resource guide of services.

Options Counseling 

  • Support in identifying and prioritizing goals and help coordinating access to public and private long-term services, such as transportation, home care, health and medical and social services.
  • Help tapping into available services such as transportation, home care, health and medical and social services.

Short Term Coordination

  • Expedited access to services and supports for individuals at risk of institutionalization of hospitalization.
  • Guidance from an ADRC Solano staff member to make a plan for you to connect to eligibility-determined, fee-for-service and/or free services and resources available in the community.

Transition Assistance

  • Support with transitioning from a hospital, nursing facility or other institution to a home, the community or another less restrictive living situation.
  • Information and access to the full range of available resources and services to support the transition process and assist with in-home care needs, including paperwork, planning and advocacy.
