• Short-term services coordination provides expedited access to services and supports for individuals at risk of institutionalization or hospitalization
   
• Priority is given to consumers who need urgent access to services and programs, without which their health and safety would be at risk

How it Works:

Short Term Coordination includes guidance from an ADRC staff member to make a plan for you to connect to eligibility-determined, fee for service and/or free services and resources available in the community.
