• Transition Assistance helps individuals who are currently in a hospital, nursing facility, or other institution and wish to transition home or the community
   
• Support includes information and access to the full range of available resources and services to support the transition process and assist with in-home care needs, including paperwork, planning and advocacy

How it Works:

Transition Assistance provides guidance from an ADRC staff member to help individuals who are living in facility care move home or to a less restrictive living situation.
